-
Where patient-reported measures information is stored

-
Protecting information in HOPE
Information in HOPE is managed in line with NSW Privacy Legislation, and the NSW Health Electronic Information Security Policy. Refer them to the NSW Health’s Patient Privacy webpage for more information.
How information is used: example script
This example helps you clearly explain privacy and security to participants in the Patient Reported Measures Program.
I want to explain how your answers are used.
What you share helps me and your care team better understand your health and plan your care. Your answers can be shared with other health professionals who are involved in your care.
Your answers are stored in the NSW Health system. You can always see your own health answers in the HOPE platform.
Feedback about your experience of care is different. That information is grouped together, has your details removed and is only used to improve services.
Sometimes de-identified information is also used for things like service planning and improvement.
