When a person first accesses the Health Outcomes and Patient Experience (HOPE) platform, they are invited to provide consent to take part in the Patient Reported Measures (PRMs) Program and for NSW Health to collect, use and disclose their information.
It's important to yarn about these key points:
- Participation is voluntary.
People choose to take part and can stop at any time. - Consent is requested at first access.
HOPE prompts people to provide consent when they first log in. - Consent can be withdrawn.
If a person withdraws consent, information already provided remains in HOPE but is kept in a de-identified form. - How information is used.
NSW Health uses PRMs to support individual care and, when de-identified, to improve health services.
Consent and use: example scripts
Example scripts help you answer questions clearly and confirm that each person understands what they are agreeing to.
Taking part in this program is completely your choice. You can say yes, no, or change your mind at any time.
When you first log in to the HOPE platform, it will ask for your consent. That means you agree to take part and for NSW Health to use your information to support your care, and that it may be used to improve health services. You can take your time to read it.
If you decide to stop later, that’s okay. Any information you’ve already shared will stay in the HOPE system, but your personal details will be removed so no one can tell it’s yours. Your de-identified information will only be used to help improve NSW Health services.
