Working with ACI networks and healthcare providers in the adoption of healthcare innovations and service improvements across NSW.

Implementation is the process of effecting change by shifting behaviours and putting the desired change into practice.

We use Redesign methodology and Accelerating Implementing Methodology in the development of flexible, patient-centred models at a state level, and the Plan, Assess, Operationalise process is used to support implementation of models of care at a site level.

    Implementation Project Management Plan Template (DOC 75.5 KB)

    The purpose of the project management plan is to provide clarity on what is to be achieved by the project (aim, objectives and scope). The plan is an agreement between the Project Lead, Sponsor, Project Team and other business unit managers associated with and/or affected by the project.
