This tool and related resources have been developed to support health services across NSW to understand their current service capacity and capability around chronic wound management, and take action towards meeting the NSW Health Leading Better Value Care Standards for chronic wound management.
The resources have been developed to support nominated Wound Management Initiative leads in NSW local health districts (LHDs) to complete the wound management self-assessment process and action planning. The tool aims to:
- identify areas of good practice and areas for improvement
- inform the development of a roadmap towards meeting the Standards
- provide a common understanding among those involved in improving wound care services.
