Quick guide to heart failure wellbeing

This fridge magnet is a tool to support people with heart failure to remain well,  recognise when they are becoming unwell and seek support from their health providers. It has:

  • daily tips
  • changes to watch out for
  • when to seek advice from health professionals or call emergency services.

Clinicians can share the magnet with patients as a reminder about self-management and ongoing care.

A version of the magnet for Aboriginal people is also available to share with Aboriginal patients.

Versions in community languages are also available.

Artwork by Dennis Golding

The Aboriginal artwork on the border of the magnet is titled ‘Deep within’. It references the pigments of land and soil between coastal and inland regions of NSW.

The earth tones of ochre and soils are interwoven through patterns of river streams and circular currents to acknowledge the connection between saltwater and freshwater people.

The artwork uses cultural motifs that reference waterholes, meeting places and gatherings, which are interpreted through continued cultural practices for community who share, exchange, learn and teach.
