Transition Care Network showcase event

This year’s network event will be held virtually, with a focus on lived experiences and how services support transition care.

We’re seeking presentations that showcase:

• the lived experiences of young people and families, and working with consumers
• making connections with clinicians across paediatric and adult health services
• building processes and models to support transition care
• using data and evidence to drive local change.

Consider sharing your work or project, no matter how big or small.

Audience

This event is for consumers and staff involved in the care of young people with chronic conditions who transition from paediatric to adult health care.
