About the Primary, Integrated and Community Health team

Primary, Integrated and Community Health aims to increase integration of healthcare delivery for people with (or at risk of) living with long-term health conditions or who have complex needs.

Team structure

The team is led by a manager and supported by an executive committee. We work to improve and better integrate care throughout the person’s entire care journey and across all providers and care settings.

Executive committee

The executive committee has diverse membership including community and primary health care professionals and consumers. The committee promotes a whole-of-system perspective, bringing a focus on integrated and primary healthcare to the ACI and NSW Health.

General Practice Advisory Group

The group provides expert general practice advice to ACI clinical networks and NSW Health on strategy and policy in primary care.

NSW Chronic Wound Taskforce

This taskforce guides and coordinates the implementation of the NSW Leading Better Value Care chronic wound management initiative.

Chronic Wound Community of Interest

The community provides NSW clinicians, managers, carers and care providers with the opportunity to:

  • share ideas
  • advise and guide the system
  • highlight locally introduced innovative solutions
  • raise issues that would benefit from a NSW system response.
