Aged health services model in NSW

It is the responsibility of the aged health system to deliver person-centred quality care to older people, their carers and families.

We explored ideal models for NSW Health to deliver aged health services, with the aim of improving the care of older people.

Three different service models are presented. These reflect different aspects of aged health services in NSW:

  • aged health service settings
  • the older person’s hospital pathway
  • service distribution with the progression of frailty.

These models present current NSW aged health services and what is needed to change to an optimal state.

The service models in this document are accompanied by a decision support tool to identify the impact on patient care, service and the aged health system, if services are transferred, reduced, changed or enhanced.
