Enhancing access to diabetes specialist care

This toolkit supports health services to improve access to diabetes expertise, build workforce capability, and support multidisciplinary and shared care for people living with diabetes.

Growing demand for diabetes care and limited availability of specialists require new ways of working.

This toolkit gives practical guidance to establish and enhance local models of care that:

  • support more connected and consistent approaches to care
  • reduce avoidable hospitalisations
  • improve health outcomes
  • make better use of health services.

It is intended for clinicians, service managers and administrators across local health districts, specialty networks and primary health networks.

This toolkit embeds a commitment to culturally safe, person-centred care for Aboriginal people, delivered in partnership with communities and the Aboriginal workforce to improve access and outcomes for people living with diabetes.
