Cystic fibrosis model of care

Survival of people with cystic fibrosis has increased in recent decades. This has resulted in an increased number of adult patients with cystic fibrosis and an increase in hospitalisation rates as disease severity progresses. As cystic fibrosis patients age, the complexity of health care increases, as well as the need to focus on multiple aspects of preventive care.

The cystic fibrosis model of care addresses the lifelong healthcare needs of people with cystic fibrosis.

The model defines opportunities across both the cystic fibrosis annual cycle of care and at specific life stages where patient-centred, flexible service delivery formats may improve access to care, timeliness of care, or better meet individual patient needs.
