Overview
The Virtual Care Exchange Forum is a highly regarded forum that provides the opportunity for participants to extend their knowledge and capability when integrating virtual care into clinical practice. It is facilitated by experts and clinical champions who openly share their knowledge and learnings (successes and failures) from integrating technology into clinical care.

The Northern Sydney Local Health District (NSLHD) - Long COVID-19 service
Having been awarded the NSLHD Exceptional People Award in the Research, Innovation, and Digital Health category. Presenters Dr James Zhang and Lorna Hatcher will share their lessons in delivering a model of care that is 100% virtual, while exploring transferability to other service settings and scalability as they extend services into Central Coast local health district.
Royal Adelaide Hospital - Telehealth high-risk foot service
Aboriginal health practitioner Joseph Agius and High-risk foot coordinator Sonja Rogasch join us from Royal Adelaide Hospital in Central Adelaide Local Health Network to discuss their telehealth high-risk foot service. The continuity-of-care approach has led to improved outcomes for diabetes-related foot complications in Aboriginal and Torres Strait Islander people.
Agency for Clinical Innovation - Virtual care in practice guide
ACI is proud to announce the release of the digital virtual care in practice guide and provide an overview of the comprehensive online resource that supports the implementation and delivery of clinical care.
Discussion
The open discussion is an opportunity for participants to access information, highlight positive advancements or to raise challenges in providing care virtually. Key representatives will be available to support this component.
