Eligibility

Patient on the waiting list for elective total hip or knee replacement; Referral from specialist for conservative management; Referral from general practitioner for chronic care interventions for osteoarthritis

Referrals from General Practitioners, Specialists and Allied Health Practitioners

Must reside within St George / Sutherland Local Government Areas

Services provided

• Multidisciplinary team includes:
  • Dietitian
  • Occupational Therapist
  • Physiotherapist
• Intervention provided:
  • Comprehensive assessment, intervention and education with coordinator and MDT
  • Prescription of home exercise program
  • Land Based exercise class
  • Development of Personalised Management Plan
  • Liaison with GP and other specialists
• Common referrals
  • Community-based exercise programs
  • Get Healthy
  • Stepping On
