Patient on the waiting list for elective total hip or knee replacement; Referral from specialist for conservative management

All patients who reside in the Ryde Area referred to Royal North Shore Hospital for appointments

Referrals accepted from a select group of General Practitioners

Referrals from Allied Health Professionals within Northern Sydney LHD

  • Multidisciplinary team includes:
    • Dietitian
    • Occupational Therapist
    • Orthotist
    • Physiotherapist
    • Rheumatologist
    • Social Worker
  • Intervention provided:
    • Comprehensive assessment, intervention and education with coordinator and MDT
    • Prescription of home exercise program
    • Development of Personalised Management Plan
    • Liaison with GP and other specialists
  • Common referrals
    • Community-based exercise programs
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health
