About this service

Age 50 years and over with minimal trauma fracture; Age 40 years and over with minimal trauma fracture and Aboriginal or Torres Strait Islander

No history of metastatic bone disease Referrals are accepted from ED, Fracture Clinic Teams, in patient wards, GPs, allied health specialists.

  • Multidisciplinary team includes:
    • General Medicine Physician
    • Physiotherapist
  • Intervention provided:
    • Comprehensive assessment, intervention and education with coordinator and MDT
    • Osteoporosis medication regimen started (if indicated)
    • Bone Health and Falls risk assessment
    • Bone Mineral Density Scan
    • Pathology service and review
    • Medical Specialist review
    • Development of Personalised Management Plan
    • Liaison with GP and other specialists
  • Common referrals
    • Community-based exercise programs
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health
    • Bone Mineral Density Scan
