Patient on the waiting list for elective total hip or knee replacement; Referral from specialist for conservative management; Referral from general practitioner for chronic care interventions for osteoarthritis

  • Multidisciplinary team includes:
    • Physiotherapist
  • Intervention provided:
    • Comprehensive assessment, intervention and education with coordinator
    • Prescription of home exercise program
    • Land Based exercise class
    • Hydrotherapy group sessions (if appropriate)
    • Group education sessions
  • Common referrals
    • Community-based exercise programs
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health
    • Get Healthy
    • Stepping On
    • My Aged Care
