Patient on the waiting list for elective total hip or knee replacement; Referral from specialist for conservative management; Referral from general practitioner for chronic care interventions for osteoarthritis
- Multidisciplinary team includes:
- Physiotherapist
- Intervention provided:
- Comprehensive assessment, intervention and education with coordinator
- Prescription of home exercise program
- Land Based exercise class
- Hydrotherapy group sessions (if appropriate)
- Group education sessions
- Common referrals
- Community-based exercise programs
- Multidisciplinary Allied Health teams within hospital
