Low bone density results with or without fractur
Prostate cancer diagnosis on ADT
Residing in Coffs Harbour, Bellingen or Macksvile LGA's
- Multidisciplinary team includes:
- Allied Health Assistant
- Geriatrician
- Physiotherapist
- Intervention provided:
- Comprehensive assessment, intervention and education with coordinator
- Bone Health and Falls risk assessment
- Bone Mineral Density Scan
- Pathology service and review
- Medical Specialist review
- Development of Personalised Management Plan
- Liaison with GP and other specialists
- Common referrals
- Community-based exercise programs
