About this service
Patient on the waiting list for elective total hip or knee replacement; Referral from specialist for conservative management; Referral from general practitioner for chronic care interventions for osteoarthritis
Referrals from General Practitioners accepted with a clinical Osteoarthritis (OA) diagnosis with the addition of OA related knee of hip joint pain
- Multidisciplinary team includes:
- Allied Health Assistant
- Dietitian
- Exercise Physiologist
- Occupational Therapist
- Physiotherapist
- Intervention provided:
- Comprehensive assessment, intervention and education with coordinator and MDT
- Prescription of home exercise program
- Land Based exercise class
- Group education sessions
- Development of Personalised Management Plan
- Liaison with GP and other specialists
- Common referrals
- Community-based exercise programs
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
