It can be helpful to have a carer or support person with you as we discuss your care needs and how to keep you safe.

How we will work together to keep you safe

So we understand what’s happening for you, please tell us as much as you can about:

  • what matters to you
  • what is worrying you
  • any recent changes
  • past and current suicidal behaviour
  • the people and things that support you
  • what helps to keep you safe.

We’ll explore your choices and options for care and support.

This will help us to understand the risks you’re facing.

If you’re comfortable, we’d also like to talk to your carer, friend, or support person. Working together, we’ll make sure you have a current safety plan.
