About this service
- Diabetes
- Foot Ulcer/wound
- Osteomyelitis/Bone Infection
- Charcot neurarthropathy deformity (either acute or chronic)
- Acute Lower limb ischemia with foot ulceration
- Gangrene of the lower limb
- Cellulitis or soft tissue infection or absess
- Peripheral Arterial Disease
- Peripheral Neuropathy
- Peripheral Vascular Disease
- Previous Amputation
Patient must reside in the Hunter New England Local Health District
Medical referral required, Patient must have an acute foot complication (key words in referral from eligibility list above)
- Multidisciplinary team includes:
- Aboriginal Health Practitioner
- Clinical Nurse Consultant
- Diabetes Educator
- Dietitian
- Endocrinologist
- Occupational Therapist
- Orthopaedic Specialist
- Orthotist
- Physiotherapist
- Podiatrist
- Registered Nurse
- Vascular Specialist
- Wound Nurse Practitioner
- Intervention provided:
- Comprehensive assessment, intervention and education with clinician and MDT
- Outreach
- Prescription of home exercise program
- Pressure off-loading footwear, devices
- Preventative and Step down Podiatry services
- Common referrals
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
Preventative /Stepdown podiatry services available:
Commonwealth Home Support Program (CHSP) services offered through current locations include: Armidale, Glen Innes, Inverell, Moree,Tamworth and Tenterfield.
Patients who are CHSP eligible can also access podiatry through Home Nursing Group (6 visits/year for $50).
Patients who have Home Care Packages Level 3 and 4 can be seen by a private podiatrist. Other services available such as EPC
Telehealth offered throughout the entire New England.
