In addition to emailing or faxing a referral, for urgent cases (including suspected osteomyelitis or acute Charcot foot) please discuss with the registrar on-call. During office hours (8.00am- 4.30pm, Mon-Fri, excluding public holidays) call the Diabetes Centre on (02) 8382 2622, and ask to speak to the Diabetes Registrar.

Out of office hours call the hospital switchboard on (02) 8382 1111 and ask to speak to the Endocrine Registrar on-call.

About this service

  • Diabetes
  • Foot Ulcer/wound
  • Osteomyelitis/Bone Infection
  • Charcot neurarthropathy deformity (either acute or chronic)
  • Acute Lower limb ischemia with foot ulceration
  • Cellulitis or soft tissue infection or absess

Patients with Diabetes aged 16 years and over with one or more of the eligibility criteria.

Areas serviced: SVHN catchment area and out of area when required

  • Multidisciplinary team includes:

    • Diabetes Educator

    • Infectious Disease Physician
    • Nephrologist
    • Occupational Therapist
    • Orthopaedic Specialist
    • Orthotist
    • Physiotherapist
    • Podiatrist
    • Psychologist
    • Rehabilitation Specialist
    • Social Worker
    • Vascular Specialist
    • Wound Nurse Practitioner
  • Intervention provided:
    • Comprehensive assessment, intervention and education with clinician and MDT
    • Pressure off-loading footwear, devices
    • Preventative and Step down Podiatry services
  • Common referrals
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health

Preventative or Step-Down services available-  Ongoing Podiatry HRFS 5 days per week in the same location

No Virtual Care available using Video and/or Telephone Wheelchair accessible
