- Diabetes
- Foot Ulcer/wound
- Osteomyelitis/Bone Infection
- Charcot neurarthropathy deformity (either acute or chronic)
- Acute Lower limb ischemia with foot ulceration
- Cellulitis or soft tissue infection or absess
- Multidisciplinary team includes:
- Aboriginal Health Worker
- Diabetes Educator
- Dietitian
- Endocrinologist
- Infectious Disease Physician
- Orthopaedic Specialist
- Orthotist
- Physiotherapist
- Podiatrist
- Social Worker
- Vascular Specialist
- Wound Nurse Practitioner
- Intervention provided:
- Comprehensive assessment, intervention and education with clinician and MDT
- Prescription of home exercise program
- Pressure off-loading footwear, devices
- Preventative and Step down Podiatry services
- Common referrals
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
A written referral from your health professional along with pathology results
Other consultative services available:: Neurologist, Hyperbaric Medicine, Plastic Surgery, Community Health Nursing
Preventative or Step-Down services: War Memorial Hospital Podiatry (if meets criteria). Aboriginal Community Health Service, La Perouse (if meets criteria).
