- Diabetes
- Foot Ulcer/wound
- Charcot neurarthropathy deformity (either acute or chronic)
- Acute Lower limb ischemia with foot ulceration
- Cellulitis or soft tissue infection or absess
- Peripheral Arterial Disease
- Peripheral Neuropathy
- Peripheral Vascular Disease
- Multidisciplinary team includes:
- Aboriginal Health Worker
- Diabetes Educator
- Dietitian
- Endocrinologist
- Orthotist
- Physiotherapist
- Podiatrist
- Rehabilitation Specialist
- Vascular Specialist
- Wound Nurse Practitioner
- Intervention provided:
- Comprehensive assessment, intervention and education with clinician and MDT
- Outreach
- Pressure off-loading footwear, devices
- Preventative and Step down Podiatry services
- Common referrals
- Community-based exercise programs
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
Preventative or ‘step-down’ podiatry services
Private podiatry is available accessible through a GP referral. If over the age of 65 (or 55 for Aboriginal people) patients may be eligible for My Aged Care Podiatry.
