  • Foot Ulcer/wound
  • Osteomyelitis/Bone Infection
  • Charcot neurarthropathy deformity (either acute or chronic)
  • Acute Lower limb ischemia with foot ulceration
  • Cellulitis or soft tissue infection or absess
  • Peripheral Arterial Disease
  • Peripheral Neuropathy
  • Peripheral Vascular Disease
  • Previous Amputation
  • Multidisciplinary team includes:
    • Aboriginal Health Worker
    • Clinical Nurse Consultant
    • Diabetes Educator
    • Dietitian
    • Endocrinologist
    • General Medicine Physician
    • Orthotist
    • Podiatrist
    • Vascular Specialist
    • Wound Nurse Practitioner
  • Intervention provided:
    • Comprehensive assessment, intervention and education with clinician and MDT
    • Pressure off-loading footwear, devices
    • Preventative and Step down Podiatry services
  • Common referrals
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health

Preventative or Step-Down services provided to:

Rheumatoid Arthritis with significant foot deformity, Renal Failure- Chronic/Dialysis, Spina Bifida, Transplant recipients, Aboriginal and/or Torres Strait Islander peoples
