About this service

  • Diabetes
  • Foot Ulcer/wound
  • Osteomyelitis/Bone Infection
  • Charcot neurarthropathy deformity (either acute or chronic)
  • Acute Lower limb ischemia with foot ulceration
  • Gangrene of the lower limb
  • Cellulitis or soft tissue infection or absess

Must reside within the Hunter New England Local Health District

Must present with an acute foot complication as listed above (key words to be included in referral)

  • Multidisciplinary team includes:
    • Diabetes Educator
    • Dietitian
    • Endocrinologist
    • Orthopaedic Specialist
    • Podiatrist
    • Vascular Specialist
  • Intervention provided:
    • Comprehensive assessment, intervention and education with clinician and MDT
    • Outreach
    • Pressure off-loading footwear, devices
    • Preventative and Step down Podiatry services
  • Common referrals
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health

Entire HNELHD, Outreach available t o Upper Hunter Sector and Tamworth in the Peel Sector

Community Nursing available inclusive of a wound clinic
