Diabetes foot assessment clinics run at Gosford, Woy Woy, Long Jetty and Wyong Hospitals and Erina Community Health Centre
About this service
- Diabetes
- Foot Ulcer/wound
- Charcot neurarthropathy deformity (either acute or chronic)
- Acute Lower limb ischemia with foot ulceration
Wound below the ankle or charcot's neuroarthropathy requiring specialist medical and multidisciplinary input
- Multidisciplinary team includes:
- Diabetes Educator
- Dietitian
- Endocrinologist
- Orthopaedic Specialist
- Orthotist
- Podiatrist
- Vascular Specialist
- Wound Nurse Practitioner
- Intervention provided:
- Comprehensive assessment, intervention and education with clinician and MDT
- Pressure off-loading footwear, devices
- Preventative and Step down Podiatry services
- Common referrals
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
