About this service

  • Diabetes
  • Foot Ulcer/wound
  • Osteomyelitis/Bone Infection
  • Charcot neurarthropathy deformity (either acute or chronic)
  • Acute Lower limb ischemia with foot ulceration
  • Gangrene of the lower limb
  • Cellulitis or soft tissue infection or absess
  • Peripheral Arterial Disease
  • Peripheral Neuropathy
  • Peripheral Vascular Disease
  • Previous Amputation
  • Multidisciplinary team includes:
    • Aboriginal Health Practitioner
    • Aboriginal Health Worker
    • Allied Health Assistant
    • Diabetes Educator
    • Dietitian
    • Endocrinologist
    • Orthopaedic Specialist
    • Orthotist
    • Podiatrist
    • Social Worker
    • Vascular Specialist
  • Intervention provided:
    • Comprehensive assessment, intervention and education with clinician and MDT
    • Outreach
    • Pressure off-loading footwear, devices
    • Preventative and Step down Podiatry services
  • Common referrals
    • Community-based exercise programs
    • Multidisciplinary Allied Health teams within hospital
    • Community based Allied Health

Preventative or ‘step-down’ podiatry services available: District and Community nursing once discharged for ongoing dressings. Private Podiatrist EPC referral.

Outreach: Outreach services are available at 4 locations – currently servicing Mudgee, Coonamble, Walgett and Lightning Ridge. This service is carried out ewvery 6 weeks with follow up 2 week telehealth consults in between.

Telehealth: Following face-to-face consult there is follow-up every fortnight via telehealth with patient and nurse on the receiving end.

No Virtual Care available using Video and/or Telephone Wheelchair accessible
