About this service
- Diabetes
- Foot Ulcer/wound
- Charcot neurarthropathy deformity (either acute or chronic)
- Acute Lower limb ischemia with foot ulceration
- Cellulitis or soft tissue infection or absess
- Peripheral Arterial Disease
- Peripheral Neuropathy
- Peripheral Vascular Disease
- Previous Amputation
Patients must reside in SLHD catchment area, unless the patient is from a rural or regional area.
Suspected or Confirmed Charcot (managed in conjunction with RPAH Diabetes Centre HRFS & Dr Mark Horsley) Vascular disease with tissue loss
Patients who have severe vascular disease may not be appropriate for these services. We invite phone calls to discuss the appropriateness for treatment of these patients.
- Multidisciplinary team includes:
- Diabetes Educator
- Endocrinologist
- Orthopaedic Specialist
- Orthotist
- Podiatrist
- Vascular Specialist
- Intervention provided:
- Comprehensive assessment, intervention and education with clinician and MDT
- Pressure off-loading footwear, devices
- Preventative and Step down Podiatry services
- Common referrals
- Multidisciplinary Allied Health teams within hospital
- Community based Allied Health
