The End of Life and Palliative Care organisational models were developed in consultation with the End of Life and Palliative Care Clinical Principles Working Group members and the ACI Evidence team.
The models of care are based on an evidence review conducted in 2020. This resource has been developed to support best practice and improve the delivery of end of life and palliative care across NSW.

The End of Life and Palliative Care Self-Assessment Tool reviews activity across the nine key actions identified in the guideline to identify areas where the service or entity is excelling in implementing the key actions and those areas where there are opportunities for improvement.

The NSW Paediatric Palliative Care Programme provides a multidisciplinary, consultative palliative care service to support patients aged 0-18 years, carers and clinicians.
South Western Sydney Local Health District
The Community Palliative Care Service in SWSLHD has implemented a triage guideline to improve efficiency and efficacy for clinicians, and support for clients.
Justice Health and Forensic Mental Health Network
The Justice Health and Forensic Mental Health Network uses a multidisciplinary approach to provide palliative care in correctional facilities in NSW.
Hornsby Ku-ring-gai Hospital Palliative Care Service
Hornsby Ku-ring-gai Hospital uses AMBER Care Bundle to provide palliative and end of life care.
Southern NSW Local Health District
Virtual care is used to support the delivery of palliative care services, enabling consultations at home or in a healthcare setting with a multidisciplinary clinical team, the patient and their family.
South Eastern NSW PHN, Southern NSW Local Health District, Capital Health Network – ACT PHN and ACT Health
The ACT and Southern NSW HealthPathways enable GPs to appropriately assess, triage and manage palliative and end of life care needs of patients.
Far West Local Health District
The Far West Local Health District specialist palliative care service provides 24/7 needs-based care to patients with life-limiting disease in the last year of life.
HammondCare
The HammondCare Extended Hours Palliative Care Service (EHPCS) provided services on the weekend and after-hours, reducing the number of acute hospitalisations, saving money and increasing HammondCare bed occupancy.
