Every rehabilitation service has their preferred rehabilitation plan template. This will be influenced by:
- the nature and context of the service
- service and team preferences
- team type
- stakeholder requirements.
The following format was developed in the NSW Health rehabilitation context. It presents a single goal, but most people will have more than one goal.
This example is not intended for use as a rehabilitation plan template, but rather to demonstrate the hierarchical relationship between goals, steps and actions when documenting a rehabilitation plan.
