Person, patient, client, consumer, participant | This toolkit uses ‘person’ to refer to anyone receiving rehabilitation care in any NSW Health setting, irrespective of age. In other clinical or support contexts these people are referred to as patients, clients, consumers, or participants. |
Goal | The term ‘goal’ is used extensively throughout the toolkit. This is consistent with current terminology used in person-centred-practice and rehabilitation literature. You are encouraged to use terminology most appropriate to the person you are working with. Substitute the word goal with alternative words or phrases. |
Steps, objectives, sub-goals | Use of objectives, sub-goals and/or steps to achieve main goals will vary across settings of care, tailored for each person. While this toolkit uses goals and steps, construct your rehabilitation plans in a way meaningful to the person and actionable by both the person and the rehabilitation team. |
Family, carer, significant others | All rehabilitation includes the other people most important to the person. This may be members of the person’s immediate or extended family, close friends who provide support and/or community members important to the person. Who should be involved throughout the rehabilitation process is a personal decision made by the person and will be influenced by their choices, their social circumstances and their cultural context. This is particularly relevant when working with Aboriginal people. Throughout this toolkit, it is assumed that family and carers will be involved as and when needed wherever reference to working collaboratively with the person is mentioned. |
