Rehabilitation services in NSW Health are:
- value-based
- person-centred
- goal-directed.
Person-centred care:
- is respectful of, and responsive to, the person’s preferences, needs and values
- ensures their values and priorities guide all clinical decisions
- supports the person to recognise and develop their own strengths and abilities to enable them to live a fulfilling life.
Delivering person-centred rehabilitation care can feel time-consuming in a busy therapeutic environment. However, it can lead to meaningful functional outcomes and quality of life for people, which can reduce their need for ongoing care.
This toolkit refers to 'working with the person'. This includes their family members and carers, as needed.

All patients should be asked if they identify as Aboriginal or Torres Strait Islander. Any Aboriginal health workers involved in the person’s care must be included in the rehabilitation care team.
