Remote patient monitoring (RPM) is a way to deliver virtual care that uses digital health technologies for the remote collection, transmission and analysis of a patient's health data. This data is transmitted to a portal, so that healthcare professionals and patients, or their family members and carers, can track and manage a patient's health conditions, symptoms and vital signs.
This guide is intended for health services and clinicians establishing and working with RPM in NSW. It is made up of two main sections:
- Core information – guidance for the implementation and delivery of RPM in both new and existing models of care, to support effective integration of RPM into clinical practice.
- Cohort-specific information – additional guidance outlining the care requirements of patient cohorts with specific health conditions, including RPM best practice and sample care pathways.
