ACI Integrated Care

Integrated care involves the provision of seamless, effective and efficient care that responds to all of a person’s needs, across physical and mental health, in partnership with the individual, their carers and family.

$120 million has been committed over four years to implement new, innovative locally-led models of integrated care across the State which will transform the NSW healthcare system.  

The Agency for Clinical Innovation (ACI) has been commissioned to lead the implementation of two streams of the NSW Integrated Care Strategy - Risk Stratification and Patient Reported Outcomes/Experience Measures. Aside from the work the ACI is doing to support the implementation of the NSW Integrated Care Strategy, the ACI keeps the patient experience at the forefront of its work as a matter of course. The ACI takes an integrated approach to all its work by engaging clinicians, consumers and managers from hospital, community health and primary health care providers to meet the needs of targeted patients and their carers, including those in rural and remote areas. Our models of care are designed to promote better healthcare across an integrated health system so that everyone can benefit.
