ACI Transition Care Network members and clinicians working in the transition space are invited to attend this workshop. The aim of the workshop is to share key learnings from Transition Care Network projects in 2022 and discuss how we turn these key learnings into actions for the Network/System to transform transition in NSW. It will be an interactive session to help shape future plans.
