Draft: Cystic Fibrosis Model of Care Review

There has been a rapid increase in survival for people with Cystic Fibrosis (CF) over recent decades which has significantly changed the demographic of the CF population, increased the complexity of care and the demand for health care services in NSW.

Clinicians and consumers identified the need to understand the current health care needs of people with CF and to identify ways that effective and patient centered care could be delivered across the lifespan, now and into the future.

The ACI Respiratory Network, Cystic Fibrosis Workgroup has developed a Model of Care that provides a flexible framework for establishing an expanded service delivery model across NSW. The service model maintains the role of CF Specialist Service Centres which deliver care or directly oversee episodes of shared care involving designated CF Satellite services, Hospital in the Home, Transplant units, General Practitioners, Palliative Care and local service providers across NSW.
