The Primary Healthcare Institute (PHI) works with clinicians, consumers and managers and brings a primary care lens to the work of ACI. We strengthen relationships across the continuum of care to advance person-centred, integrated and sustainable healthcare for NSW.

Chronic Wound Management
The Leading Better Value Care Chronic Wound Initiative aims to streamline the way we provide care for people living with chronic wounds to improve the experience of receiving and providing care, enhance outcomes for patients and optimise the use of resources.

Consumer Enablement
Consumer enablement is the extent to which people understand their health conditions and have the confidence, skills, knowledge and ability to actively manage their health and wellbeing and access the healthcare services they need. The PHI is working to build capability and embed consumer enablement principles and approaches into clinicians’ daily practice across the system. See also Consumer Enablement Guide and Consumer Enablement Spotlight Series.

Navigating the Healthcare Neighbourhood
The Navigating the Healthcare Neighbourhood project includes the socialisation, implementation, spread and sustainability of the Healthcare Neighbourhood model.
