Review of a self-management Action Plan for Chronic Heart Failure

The Agency for Clinical Innovation (ACI) Cardiovascular Clinical Expert Advisory Group, a sub-committee of the Cardiac Network has developed a draft Chronic Heart Failure (CHF) Action Plan to support self-management for patients with CHF.

The Action Plan supports patients to recognise when their symptoms are worsening and prompts them to call their CHF nurse or doctor to seek advice rather than presenting directly to the Emergency Department. Clinicians will provide the plan to patients on discharge and explain how it should be used.

Feedback on the Action Plan has been incorporated from the Consumers Health Forum and specific advice will be obtained from consumers with CHF through a series of focus groups in both metropolitan and rural locations.

The document is part of a suite of resources to support the NSW Ministry of Health (MoH) Leading Better Value Care (LBVC) program.

Feedback would be most welcome until close of business on Friday, 14 February 2020

  • language
  • diagrams
  • colours
  • font size
  • ease of reading
  • whether patients will understand the prompts to contact their health professional or call an ambulance if they are experiencing cardiac symptoms
  • any other comments.
