Acute Care of the Elderly (ACE) Model of Care

This model is based on Hornsby Ku-ring-gai Health Service's Acute Care of the Elderly (ACE) Model of Care. ACE is a shared care model between physicians, geriatricians and their teams. It integrates comprehensive geriatric assessment into the optimal medical and nursing care of patients in a multidisciplinary environment. The multidisciplinary team focuses on providing the right care at the right time, patient function and patient safety. Consideration is given to the entire patient journey from admission, through the hospital and discharge to appropriate community resources.

This model of care can help meet the special requirements of the health system's most frequent users, the elderly.

ACE Patient Journey

Step 1: Lorna assessed by Triage Nurse upon arrival. Seen by ASET. Lorna identified as an ACE patientStep 2: Lorna is admitted to ACE ward under care of Cardiologist and GeriatricianStep 3: Comprehensive Geriatric assessment in liaison with GP reveals short term memory loss, limb weakness and depressionStep 4: ACE Team provide holistic careStep 5: On discharge Lorna goes homeStep 6: Lorna gets help with her medication, socialising and meals. After six months at home she has no recurrence of symptoms
